I agree that the rules as written don't specify you have to spend an action to exit a car before opening a door. And that's fine. Maybe the game designers feel that spending an action is enough payment/penalty to allow a player to exit the car, open the door, and get back into the car.
But it seems a little silly to defend the idea of opening a locked door from within a car. Most doors to public buildings, office buildings, shops, etc. open outward. If you drive the car up against it, it won't open even if it's unlocked!!
Don't get me wrong, I agree with the rules interpretation, but I can't get on board with the idea that it wouldn't be more than a little "tricky" to open a locked door from inside a car using an axe. :)
